Please answer the following questions to make sure you qualify to take part in
the Free Shade Tree Pilot Program before completing the enclosed application.



• Are you a San Jose residential electric customer of Pacific Gas and Electric
with a window or central air conditioner?

• Do you own the single family home where the tree will be planted or, if not,
will your property owner sign and agree to the tree being planted?

• Will you be available between now and April 2009 for up to two
home visits with an Our City Forest representative to determine the most
suitable Shade Tree selection and location for your home and to plant your tree?

• Will you carefully follow the Tree Planting and Care Guide instructions to
help make sure your tree grows to maturity?

• After your FREE SHADE TREE is planted, will you agree to a tree inspection
within the first 3 months if your tree is selected for inspection?
